Stour Outreach Community Team (SCOT)

The SCOT team is in place to support our more vulnerable patients. Patients are referred to the team by the clinical team if help is needed for a patient to live independently in their own home, and more specifically to avoid an unexpected emergency admission. The team will make special arrangements so that vulnerable people have fair and prompt access to appropriate care.
